DOT Physical Exams are available through your employer with an active Employer Paid Services account. Your employer will provide you with an Employer Authorization form with their requested services selected, in most cases they will schedule the exam for you.
You can still obtain a DOT Physical Exam on your own as a self-pay patient, you will reach out to the clinic of choice to schedule via a phone call to the clinic, those exams will be considered self-pay with payment expected at the visit.
Keeping drivers safe is a priority. Federal law requires drivers of commercial motor vehicles to receive regular physical examinations. These exams are defined as “Department of Transportation Medical Examinations.” DOT physicals are highly regulated for drivers’ safety and all medical examiners performing Department of Transportation physicals (DOT) are trained and certified by the Federal Motor Carrier Safety Administration (FMCSA) and stay current on all DOT regulations and testing protocols.
A DOT Physical Exam determines whether a driver is physically able to safely operate a commercial motor vehicle. The physical exam evaluates a driver’s medical history, vision, hearing, blood pressure, respiratory system, & physical attributes. Drivers may be medically cleared for up to two years, with some exceptions that are determined by elements such as blood pressure and glucose levels.
DOT physicals are performed at our locations during regular business hours. Scheduling your visit is recommended to make sure that a DOT certified medical examiner is available at your choice location. Employers are encouraged to schedule your appointment with the designated contact person to avoid extended wait times or potential exposures to illnesses in the urgent care environment.

There are a few things that you can do to make your time here quick and seamless during your DOT physical exam:
- Present a current Drivers License at check in – copies or photos on your phone will not be accepted
- Present your Employer Authorization Request for the Exam at check in
- Complete the driver’s portion of the Medical Examination Report prior to your visit.
- Bring a complete list of ALL medications, including the name of the prescribing provider.
- Bring all supporting documents needed at your exam, for example, recent labs if diabetic, medical reports from treating physicians for current medical conditions. Additional information is available on the FMCSA website.
Insurance does not cover this exam. We accept cash, checks, or credit cards at the time of service.
Employer Paid Clients with active accounts will be billed as arranged. Patients who are not a part of a EPS account will be expected to pay their visit/time of service.
