Location: A variety of sites in the community
Sentara supports patients and families diagnosed with diabetes with a comprehensive team of resources. Available team members include health care providers, diabetes care and education specialists and registered dietitians. They collaborate closely with you to help you navigate diabetes care, including maintaining your blood sugar within the desired range. In the event you have additional health concerns, you can also be referred to specialists for additional support with concerns such as eye, kidney and foot care.
Outpatient individual services
- Prediabetes (A1c 5.7% to 6.4%):
- Diabetes Prevention Program (DPP)
- In-person and virtual meetings using CDC-approved National DPP curriculum to assist in creating healthy lifestyle habits to prevent or delay the onset of diabetes
- Diabetes Prevention Program (DPP)
- Diabetes (A1c 6.5% or greater):
- Diabetes education*
- Individual visits with diabetes educators to assist in self-management education and support for individuals with diabetes and people supporting them
- Diabetes Self-Management Education and Support (DSMES) group class series*
- Four group classes held weekly focusing on healthy eating, being active, taking medication, monitoring, problem-solving, reducing risks, and healthy coping

- Gestational Diabetes:

- Two-hour group class held weekly to provide education and support in the management of gestational diabetes for individuals with gestational diabetes and people supporting them

- Two-hour group class held on the 1 st and 4th Wednesday of each month to provide education and support in the management of gestational diabetes for individual with gestational diabetes and persons supporting them

How to register for outpatient services
Have you found a class that’s right for you? For information, registration and appointments, call the Diabetes Education Department at (540) 689-1001. Classes meet at Sentara RMH Medical Center unless otherwise specified.
You will need a physician referral to participate in diabetes education (individual or group class series) and gestational diabetes education classes. If you see a Sentara Medical Group provider, ask them about completing an electronic referral form. Non-Sentara providers may fax a written referral to 1-844-648-0733.
