Diabetes Class Series - 4 Week Self-Management

A referral is necessary from the participant's healthcare provider in order to sign up for group classes. In the class series participants will learn more about self-management of diabetes and gain valuable support in a group setting. Sessions will be led by a Diabetes Care & Education Specialist and registered dietitian to provide a deeper understanding of diabetes with accompanying resource materials on topics to include healthy eating, being active, monitoring blood sugar, taking medications, healthy coping, and reducing risks. The group classes are based on providing knowledge and skills that will aid in developing individualized action plans for making healthy behavior changes to aid in/optimize diabetes self-management and reaching their desired level of management. Each class series (offered throughout the year) consists of four weekly classes held on Wednesdays. Daytime and evening class times are available throughout the year.
