Sentara Healthcare conducts community health needs assessments every three years for our inpatient hospitals and outpatient surgical centers in the areas that we serve. These assessments were developed by listening to the voices of the community and provide valuable information about health and health related problems that influence individuals’ overall quality of life. The assessments blend community input with various data points to help us understand disparities that exists in vulnerable populations.

2025 Community Health Needs Assessments

This community health needs assessment was completed for the service area of Camden, Currituck, Pasquotank, and Perquiman's counties in northeastern North Carolina.

This joint community health needs assessment report was completed in collaboration with Sentara CarePlex Hospital, CarePlex Orthopaedic Ambulatory Surgery Center and Port Warwick Surgery Center, which have the identical service areas of the cities of Hampton and Newport News.

The joint 2025 MAPP2Health report was completed in collaboration with Blue Ridge Health District, UVA Health, Sentara Martha Jefferson Hospital, and Sentara Martha Jefferson Outpatient Surgery Center, which have the identical service areas of the City of Charlottesville and Albemarle, Fluvanna, Greene, Louisa, and Nelson counties. The joint 2025 MAPP2Health report will be used as the 2025 community health needs assessment for Sentara Martha Jefferson Hospital, and Sentara Martha Jefferson Outpatient Surgery Center.

This joint community health needs assessment was completed in collaboration with the Hospital for Extended Recovery, which have the identical service areas of the cities of Chesapeake, Norfolk, Portsmouth, Suffolk, and Virginia Beach.

This joint Community Health Needs Assessment was completed in collaboration with the Community Healthcare Coalition of Greater Prince William, Prince William Health District, Sentara Northern Virginia Medical Center, Lake Ridge Ambulatory Surgery Center, and Northern Virginia Heart and Vascular Center, which have the identical service areas of Prince William County and the cities of Manassas and Manassas Park.

This joint report was completed in collaboration between Sentara Obici Hospital, Sentara BelleHarbour Ambulatory Surgery, Ambulatory Surgery Center, and the Obici Healthcare Foundation, which have the identical service areas of the cities of Franklin and Suffolk, and the counties of Isle of Wight, Southampton, Surry, Sussex, and Gates (North Carolina).

This joint community health needs assessment report was completed in collaboration with Sentara Princess Anne Hospital, Sentara Virginia Beach General Hospital, Princess Anne Ambulatory Surgery Center, and Virginia Beach Ambulatory Surgery Center, which have the identical service area of Virginia Beach city.

This community health needs assessment was completed for the service area of Harrisonburg and Staunton cities, surrounded by rural counties of Augusta, Page, Rockingham and Shenandoah counties; as well as West Virginia’s Pendleton County.

This joint Community Health Needs Assessment report was completed in collaboration between Sentara Williamsburg Regional Medical Center and Surgical Suites of Coastal Virginia, which have the identical services areas of the city of Williamsburg, and the counties of Gloucester, James City, King and Queen, New Kent, and York.
