Find COVID-19 Vaccine Status and Test Results in Sentara MyChart
- Log in to your Sentara MyChart account (Web or Mobile)
- Access Test Results or COVID-19 from the menu
- Locate your COVID-19 test result from either feature
- Log in to your Sentara MyChart account (Web)
- Access Test Results or COVID-19 from the menu
- Locate your COVID-19 test result from either feature
- Select your COVID-19 test result
- Select the Printer icon to print or save your result as a PDF
The COVID-19 section in Sentara MyChart provides information about your COVID-19 vaccination and testing status all in one place (this information must be in your Sentara medical record in order to display). You can easily display, export or generate a QR code from this section to provide proof of vaccination status or your COVID-19 test results.
From the main menu, select "COVID-19" to open this section. There is also a shortcut in the top section of the home screen.
From the COVID-19 section, you will have the ability to do the following:
- Quickly view your vaccination status and any recent COVID-19 test results.
- Download proof of vaccination into a PDF.
- Generate a QR code (SMART Health Card) for vaccination or test results that can be scanned or printed. The codes can be scanned right from your mobile device by anyone who requires proof of vaccination or test result and has a scanner.
- Export information from the QR codes to another app, such as a digital Health Wallet.
- Get helpful resources about COVID-19 from the CDC.
Please Note: Your vaccination status and number of doses received in this section is dependent upon the information Sentara Healthcare has in your electronic health record. The vaccination status will show the status of the primary vaccination series (one dose for Johnson & Johnson and two doses for Moderna or Pfizer) along with booster doses when applicable.
- If on mobile, be sure you have the latest version
- Log in to your Sentara MyChart account
- Access the COVID-19 feature from the menu
- Select QR codes
- This will open a new screen with your QR code(s)
- Be sure you have the latest version of the Sentara MyChart app on your device; you may need to check for updates through the app store.
- Log in to your Sentara MyChart account.
- From the Menu, go to My Record and select COVID-19.
- Click on the Download/Export option and then download vaccination PDF (on the Sentara MyChart mobile app) or Download (on the Sentara MyChart website). You will be able to save the PDF to your mobile device (or computer), or print/email as needed.
To export your QR codes into a Health Wallet or similar app, follow the steps noted below.
iPhone Users:
- Confirm you have iOS version 15 or higher.
- Log in to your Sentara MyChart account.
- Access the COVID-19 feature from the menu
- Select QR Codes
- Tap the 'Add to Apple Wallet & Health' button
- With iOS version 15.1 or higher
Android Users:
- To export your QR code, you will need to download a separate Health Wallet mobile app on your device. Examples: Common Health and Clear Health Pass.
- Log in to your Sentara MyChart account, go to My Record and select COVID-19.
- Select the Download/Export option.
- Your QR code will export to the health wallet app you downloaded in step 1.
Health Wallets are apps that allow you to do the following (but are not limited to):
- Store your most recent COVID-19 information in a digital wallet
- Present your COVID-19 information offline when needed
- Keep healthcare information from various sources in one app
A vaccine you received outside of Sentara may not be in your Sentara medical record. Your Sentara MyChart account is only able to generate QR codes for vaccinations that were administered at Sentara, that our staff has verified and documented historically, or those that have been reconciled through the Virginia Department of Health VIIS (Virginia Immunization Information system) query process. If you do not see your vaccine in your Sentara MyChart account, you will have the option to query the Virginia Department of Health system to try to pull that information in. If no data is found, you will be presented the option to manually enter your data, which will be reviewed by a clinical staff member and reconciled to your chart as appropriate.
Your Sentara MyChart account will only show QR codes for a completed COVID-19 test result that was completed at a Sentara facility. Testing completed outside of Sentara will not be seen in your Sentara MyChart account.
Your vaccination status and number of doses displayed in the COVID-19 section may vary depending on what our system says you are eligible to receive. Please note eligibility and recommendations for third and booster doses continue to change. We will do our best to reflect these recommendations for eligibility based on what is documented in your medical record. Booster doses completed at facilities outside of Sentara will need to be added to your Sentara chart in order to show in your Sentara MyChart account. You will see an option to query the Virginia Department of Health for booster doses as well or submit manually if no data is found.
- A QR code is a scannable barcode that can be read by another device or computer.
- The QR code created in your Sentara MyChart account is generated according to an open industry standard called SMART Health Cards. These cards contain verified information and only what is relevant to COVID-19. More information on Smart Health Cards can be found here: https://smarthealth.cards/en/faq.html.
- Your QR code is another way of providing proof of your COVID-19 vaccination or test status at places where it may be required (such as for travel, work, concerts or other public events). Your QR code can be scanned at one of these venues instead of having to keep up with a paper CDC vaccine card or report that can easily be lost or damaged.
- When scanned the QR code will display your legal name, date of birth, vaccination status and/or most recent COVID-19 test result as documented in your medical chart at Sentara. It will also show that your QR code was generated from a verified issuer according to VCI.org (Vaccination Credential Initiative).
- VCI™ is a voluntary coalition of public and private organizations committed to empowering individuals access to a trustworthy and verifiable copy of their vaccination records in digital or paper form using open, interoperable standards.
